First Visit

Your First Visit

ONLINE FORMS

Your first Chiropractic visit

Please arrive 10 – 15 minutes early for your appointment. This allows you time to complete our New Patient paperwork, for our team to create your personal file, and for you to relax in our reception area.

You will be called through to your Chiropractor for your one-on-one consultation. Your Chiropractor will greet you, refer to your completed case history questionnaire and guide you through a process of discussing your health concerns, current injuries, and relevant medical history.

Your Chiropractor will listen, take notes, and get an understanding of what is prompting you to seek chiropractic care. Your Chiropractor will then discuss whether chiropractic can assist you with your health concerns. If chiropractic care seems appropriate, and you freely consent to care, your chiropractor will then conduct a thorough initial examination.

Your first Applied Kinesiology visit

As your initial appointment is to assess your health status and set new goals it would be appreciated if you could bring along any medications and supplements you are currently taking and any relevant medical reports or blood test results. This will help your AK chiropractor to accurately determine your current levels of health and determine your relevant health background.

The above information on your first chiropractic visit also applies to your first applied kinesiology visit if you’re not already a practice member at Roughan Chiropractic.

It is helpful for our AK Chiropractors to know your specific blood type (O, A, B or AB). If you don’t already know your blood type, please inform us.  We can do a simple test to find out before your appointment if we are aware. One of our Chiropractic Assistants will carry out a simple procedure involving a pinprick to your finger to determine your blood type.

Our Reception area

Our reception area is designed with your comfort in mind and to relax before your appointment. We keep the room temperature warm in the colder months with our cosy fire, and refreshing and cool in the hot summer months. We have a filtered water dispenser, a range of current magazines available and a copy of this morning’s newspaper.

Bathroom facilities are available through the hallway with disabled access and a parent’s change table.

Pre-consultation Instructions

Your appointment time is reserved for you. If for some reason you need to reschedule your appointment, we require 24hrs notice. This primarily is in consideration for others, and allows us to schedule in patients from our waiting list.

If you have further questions regarding your consultation or our practice, please call us on 03 365 7828. We would love to talk to you.

New Patient Forms

Please either print out new patient forms and bring with you or email them through prior to your first visit.
